Job Description

The incumbent in this position is responsible for developing and implementing a broad-based, comprehensive health promotion and disease prevention program for the Seminole Tribe of Florida’s Integrative Health Education Program. The individual identifies health education needs, concerns and resources through assessment methods and prepares a written Health Education and Diabetes Program Plan, which addresses areas of need. The incumbent assists with the assessment and evaluation of existing diabetes program as well as all aspects related to the accreditation process, including diabetes care, education and over-all quality improvement process measures. Master’s Degree and current health occupations license in Health Education, Community or Public Health, Exercise or Biologic Sciences, Nutrition, Education, Public Relations, Behavioral Science or Communications (such as Registered Nurse, Physician Assistant, Physician, or Master’s level health based credential) is required. Specialized training and education as a Certified Diabetes Education and Care Specialist (CDECS) or equivalency is required. A minimum of five (5) years of professional experience providing health education and diabetes care and management services is required. Prior experience in a supervisory or managerial capacity is preferred. Prior knowledge of and experience working with federally funded Tribal and urban Indian healthcare clinics, is required. Knowledge of principles, practices and procedures regarding consultation and healthcare delivery to American Indian /Alaska Native population, and grants management processes and procedures are required. Thorough knowledge of current evidence-based practices, and treatment modalities to prevent, manage and treat Type 2 diabetes and its complications in American Indian /Alaska Native patients. Ability to function as part of a multi-disciplinary team. Demonstrate proficiency in the use of the Diabetes Management (DM) package to maintain DM registries and iCare panels of DM/preDM patients. Demonstrate proficiency in Microsoft Office software. Possession of a valid Florida Driver’s License is required. Demonstrate excellent interpersonal and communication skills. Demonstrate excellent written and verbal communication skills. The incumbent must possess a valid Florida Driver’s License. Ability to travel and work a flexible schedule including evenings, weekends and holidays.
